Office Policies & Procedures
New patients:
Please arrive 10 minutes prior to your appointment time with your insurance card and driver's license. Please bring in medical records that you may have, including vaccine records.
What happens during a well child visit:
Prior to your appointment, you will receive documents via email or text to complete such as pre-visit questions and developmental questionnaires appropriate for your child's age. We will check all of your child’s vital signs including height, weight, blood pressure (starting at 3 years), and a head circumference (from birth until the 2nd birthday). These will be plotted on a growth chart and reviewed with you by your provider. Your provider will discuss age appropriate expectations for growth and development, perform an assessment and physical examination of your child, and allow you an opportunity to ask questions. At the end of your time with the provider we will perform any necessary labs and immunizations agreed upon by you and your provider during your discussion.
Consent to Treatment:
If someone other than the parent/guardian (ex. Sibling's over the age of 18, babysitter/nanny, grandparent, etc.) will be bringing the child to their visit, new or established, please let us know so that we can send a Consent to Treat form prior to the appointment.
Late for Appointments:
If you arrive more than 10 minutes after your scheduled appointment time, we reserve the right to reschedule your appointment.
Cancellations and No Shows:
We understand emergencies and illnesses are not planned and are often unavoidable. A no show or late cancellation fee may be applied to your account. Two or more missed visits within a 12 month period may lead to dismissal from our practice.
Payments:
Financial PolicyWe accept cash and major credit cards. All copays are due prior to the visit at check-in and are not billable. Deductibles must be paid the day of the visit. We offer a discounted self-pay. For balances over $50, interest-free payment plans are available at any time through our billing department. Please call our office at 469-290-3208, option 6 to discuss. Failure to keep your account current may lead to the inability to schedule appointments until a payment plan has been arranged.
Messages Left for the Providers:
If you need to reach a provider during the business day, please leave a portal message or call and leave a message with a member of our front office or triage staff. All messages will be returned accordingly. Messages that are left before 4 pm on a business day will still be returned after hours. Please remember that the providers are busy during the day with other families. If you feel that your message is of urgency, please schedule an appointment.
Referrals:
We will happily refer you to specialists based on your child’s medical conditions. Ideally, this should be done during the office visit when we address that specific concern. If you have not been seen by our providers for this condition, we may ask that you make an appointment with our office before requesting the referral. If a referral is requested, we will send a referral request form with information about the referral including the doctor’s full name, address, telephone number, fax number, specialty and reason for the referral. Please allow five (3) business days for the office to complete your referral request.
Emergent Care:
We operate by appointment only and do not see walk-ins, We do our best to accommodate all same day sick needs. If you feel that a need is immediate/urgent, and you cannot wait until our next available appointment time, we recommend the Children’s PM Pediatric Urgent Care or Cook Children's Urgent Care. The notes from a visit at the Children’s and Cook's will be available to your child’s provider for review and follow up if needed.
After Hours Medical Advice:
If you call our office between 5:00 pm and 8:30 am M-F and all day on the weekends and holidays, you can request to be directed to our after hours service, Triage 4 Pediatrics where a registered nurse can make recommendations on home management or referral to urgent care or emergency room if needed. There is a $30 charge for this service.
